PREPARTICIPATION PHYSICAL EVALUATION | Ohio High School Athletic Association – 2022-2023

Open to all Tri-Valley District girls entering the 6th, 7th or 8th grade for the 2023-2024 school year.

Participants must not be 15 years of age prior to August 1, 2023.
