My name is Anne Slaboden and I am pleased to be the School Nurse for Tri-Valley Schools. I received my Bachelor of Science in Nursing at Ohio Wesleyan Riverside School of Nursing.I have been a nurse for 22 years and have worked in all areas of nursing, requiring different skills.  I became the nurse for Tri-Valley schools in November 2009.  The most important job of a school nurse is to assist students in attaining and maintaining their optimum level of health so that they can grow physically, emotionally and academically.  Nurses in schools work in conjunction with parents who have the primary responsibility for the health of their children.

My goals are threefold, to keep our students safe and healthy, to manage health problems as they arise, and to be a resource and advocate for families in our community.  To help me achieve these goals please keep me updated of any changes regarding your child's health and be sure to inform the office staff if there are changes in your emergency contact information.

A list of screenings, forms and other health information provided for your child are located on the right.  I hope the information is helpful.

-Fluoride Mouth Rinse Program for Elementary Buildings (Parental Consent Required)
-Immunization Reviews all Year

Fall

-
Vision and Hearing Screenings for K, 1, 3, 5, 7, 9, new students, and other students as identified by teachers.

Winter

-Mobile Dentist for Elementary Buildings (Parental Notification)
-Scoliosis Screenings for 5th-7th graders


Spring

-Puberty/Hygiene Program for 5th Graders (Parental Consent Required)
-Kindergarten Registration and Screenings (Vision and Hearing)


*Reminder:  If your child has medication in the school clinic at the end of the school year, it will be disposed of as directed by the district medication policy.  Please arrange for someone to pick-up the medication if you do not want it destroyed.  No medication will be given to students to transport home.
